93.005:  Project Grants for Facilities to Improve the Health Status of Minority Populations

93.005 ELIGIBILITY REQUIREMENTS:

Applicant Eligibility:  Public and private nonprofit entities.

Beneficiary Eligibility:  Members of the minority and disadvantaged groups: Blacks; Hispanics; American Indians; Alaska Natives; Asian/American Pacific Islanders; or subgroups of these.

Credentials/Documentation:  Proof of nonprofit status is required as part of the application submission.

93.005 APPLICATION AND AWARD PROCESS:

Preapplication Coordination:  This program does not require preapplication coordination. This program is eligible for coverage under E.O. 12372, "Intergovernmental Review of Federal Programs." An applicant should consult the office or official designated as the single point of contact in the State for more information on the process the State requires to be filed with applying for assistance, if the State has selected the program for review. This program is also covered by the Public Health System Reporting requirements. An Environmental analysis may be required for project activity, in compliance with the National Environmental Policy Act of 1969.

Application Procedure:  Requests for the standard PHS application form (SF PHS 1561-1, revised May 1996) and instructions for submission should be directed to Mrs. Carolyn A. Williams, Grants Management Officer, Office of Minority Health, Office of Public Health and Science, Rockwall II Bldg., Suite 1102, Rockville, MD 20852. Phone: (301) 227-8758. A signed original and two copies of the application are required. Applications are screened upon receipt for completeness, responsiveness and conformance to the program announcement. Those applications judged to be unacceptable based on this initial screening will be returned. Accepted applications will be reviewed for technical merit by an ad hoc review panel composed of Federal and nonfederal experts who make funding recommendations to the Deputy Assistant Secretary for Minority Health.

Award Procedure:  After considering the recommendations of the review panel, the Deputy Assistant Secretary for Minority Health makes final decision on funding projects and a notice of grant award is issued.

Deadlines:  Contact Headquarters Office listed below for applications deadlines.

Range of Approval/Disapproval Time:  From 90 to 120 days.

Appeals:  None.

Renewals:  None.

Criteria for Selecting Proposals:  Proposals are evaluated on the extent of need and demand for the project; the extent to which the project will allow for the delivery of the most needed health care services to disadvantaged populations; the reasonableness of proposed costs.

Examples of Funded Projects:  Construction of National Primary Care Center; construction and renovation of facility for the promotion of the wellness of African-American women; construction of a facility to enhance educational programs in the pre-health/health and regulatory sciences programs.

Range and Average of Financial Assistance:  It is anticipated that awards will be at a minimum $1,000,000.

OMH issued two awards in fiscal year 1999. It is anticipated that no awards will be issued in fiscal year 2000 or fiscal year 2001.

93.005 ASSISTANCE CONSIDERATIONS:

Formula and Matching Requirements:  This program has no statutory formula or matching requirements.

Length and Time Phasing of Assistance:  Payment of grant funds will based on percentage of total project costs incurred. Requests for reimbursement should coincide with submission of construction contractor's request for payment.

Uses and Use Restrictions:  All funds awarded should be expended solely for carrying out approved projects in accordance with the provisions of the Omnibus Consolidated Appropriations Act for fiscal year 1997.

93.005 POST ASSISTANCE REQUIREMENTS:

Reports:  None.

Audits:  "In accordance with the provisions of OMB Circular No. A-133 (Revised, June 24, 1997), Audits of States, Local Governments, and Non-Profit Organizations, nonfederal entities that receive financial assistance of $300,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $300,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133." In addition, grants and cooperative agreements are subject to inspection and audits by DHHS and other Federal government officials.

Records:  Financial records, supporting documents, statistical records, and all other records pertinent to a grant shall be retained for a minimum of 3 years, or longer pending completion and resolution of any audit findings.
