CATALOG OF FEDERAL DOMESTIC ASSISTANCE

93.051:  New Demonstration Grants to States with Respect to Alzheimer's Disease

Objectives:  To assist States in creating a system of dementia care for persons with Alzheimer's Disease and their families, by carrying out demonstration projects for planning, establishing, and operating programs for: (1) Program Development: Coordinate with public and private organizations the development and operations of diagnosis, treatment, care management, respite care, legal counseling and education services provided with the State to: (a) Individuals with Alzheimer's disease and related disorders; (b) families of those individuals; and (c) care-giving providers of those individuals; (2) Service Delivery: Provide home health care, personal care, day care companion services, short-term care in health facilities, and other respite care for individuals with Alzheimer's disease and related disorders, that are living in single family homes or congregate setting (not less than 50 percent of the grant should be expended for (Section 2 services); (3) Improving Access: Assist with improving access of persons with Alzheimer's Disease to home-based or community-based long-term care services, particularly for such persons who are members of racial or ethnic minority groups having limited proficiency in speaking the English language, or living in rural areas; (4) Information Dissemination: Provide information on: (a) diagnostic, treatment, and related services available to individuals with Alzheimer's Disease or related disorders and their families; (b) sources of assistance in obtaining such services, including assistance under entitlement programs; (c) the legal rights of individuals with Alzheimer's Disease and related disorders and their families; and (d) organizations established for these individuals and their families. This program will also provide information to the general public.

93.051 ELIGIBILITY REQUIREMENTS:

Applicant Eligibility:  State government agencies are eligible for grant awards.

Beneficiary Eligibility:  (1) Individuals with Alzheimer's disease and related disorders; (2) families of those individuals; and (3) care providers of those individuals.

Credentials/Documentation:  Applicants must submit: (1) A description of intended use of the grant; (2) information describing programs and activities to be supported; (3) the number of individuals receiving services and the cost of services; and (4) a plan describing coordination of such services. Costs will be determined in accordance with OMB Circular No. A-87 for State and local Governments.

93.051 APPLICATION AND AWARD PROCESS:

Preapplication Coordination:  This program is eligible for coverage under E.0. 12372, "Intergovernmental Review of Federal Programs." An applicant should consult the office or official designated as the single point of contact in his or her State for more information on the process the State requires to be followed in applying for assistance, if the State has selected the program for review.

Application Procedure:  Grant Application for Federal Assistance (Nonconstruction Programs) are in accordance with program announcements developed by the Administration on Aging (AoA) and will be published in the Federal Register. Current grantees are in the first year of a 2-year project period. As specified in the program announcement, application forms and instructions may be obtained by writing to the Administration on Aging, Grants Division, 330 Independence Ave., SW., Cohen Bldg., 4th Floor, Washington, DC 20201.

Award Procedure:  After review and approval, notification of award will be issued to the grantee, along with appropriate notification to the public.

Deadlines:  As specified in guidelines published in the Federal Register.

Range of Approval/Disapproval Time:  About 60 to 90 days from closing date or receipt of application.

Appeals:  None.

Renewals:  As outlined in the program announcement published in the Federal Register.

Criteria for Selecting Proposals:  A grant may not be made to a State unless the State has submitted an application for the grant. Each application shall contain a description for the purposes for which the State expects to expend grant funds. The description of purposes shall include: (1) Information relating to the programs and activities to be supported and the services to be provided; (2) the number of individuals that will receive services under the grant; (3) a description of intended expenditures, including the average costs of providing services to each individual; and (4) a description of the manner in which the programs and activities will be coordinated with any similar programs and activities of public and private entities. The application shall provide assurances of compliance documentation of required match, and contain specific information and agreements as required by the Administration on Aging.

Range and Average of Financial Assistance:  From $217,000 to $490,000; $340,000.

93.051 PROGRAM ACCOMPLISHMENTS:

For fiscal year 1999, continuing grants were given to the 15 States from fiscal year 1998. A new grant competition for 14 to 20 State awards will be held in fiscal year 2000. It is estimated that those States selected for funding in fiscal year 2000 will be awarded continuation funding in fiscal year 2001. National program evaluation activities are in progress. During the first 5 years of program implementation, over 8,000 client families have received direct services and about 650,000 units of outreach, including educational seminars and workshops have been provided to family caregivers, community service providers, and volunteers. Nearly 50 percent of the clients served were minorities and over 53 percent of those served resided in rural areas. Over 43 percent of those served had not used any formal services prior to this program.

93.051 ASSISTANCE CONSIDERATIONS:

Formula and Matching Requirements:  Matching requirements are as follows: first year of the award, 75 percent Federal and 25 percent nonfederal matching requirement; second year, 35 percent nonfederal; and third and subsequent years, 45 percent nonfederal. The nonfederal contribution may be in the form of cash or in-kind contributions, including plant, equipment, or services.

Length and Time Phasing of Assistance:  Current grantees are in the first of a 2-year project period. Length of future awards will be as published with the program announcement in the Federal Register.

Uses and Use Restrictions:  (1) Not less than 50 percent of the grant must be spent on home health care, personal care, day care, companion services, short-term care in health facilities, and other respite care to individuals with Alzheimer's disease and related disorders. (2) Grant funds cannot be used to replace existing, State, Federal, or private insurance funds. (3) Matching funds are required. (4) Not more than 10 percent of the grant will be expended for administrative expenses. (5) Current grantees are in year 1 of a 2-year project period.

93.051 POST ASSISTANCE REQUIREMENTS:

Reports:  Financial status reports and program progress reports are required semi-annually, including a final report at the end of each grant year.

Audits:  In accordance with the provisions of OMB Circular No. A-133, (Revised, June 24, 1997), "Audits of State and Local Governments, and Non-Profit Organizations," nonfederal entities that receive financial assistance of $300,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $300,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in OMB Circular No. A-133.

Records:  Financial records must be kept available for 3 years after submission of expenditure reports and 5 years after final disposition of non-expendable property in accordance with the regulations at 45 CFR 74, Subpart D.
