93.215:  Hansen's Disease National Ambulatory Care Program

Objectives:  To provide ambulatory Hansen's disease care and treatment services to any person diagnosed with the disease who needs and requests such care and treatment, screening and testing of contacts of patients diagnosed with Hansen's disease.

93.215 ELIGIBILITY REQUIREMENTS:

Applicant Eligibility:  Existing health care facilities are eligible to apply.

Beneficiary Eligibility:  Persons diagnosed or suspected of having Hansen's disease and their contacts.

Credentials/Documentation:  Applicants must demonstrate willingness to continue in the Hansen's disease program at a level of service about the same as the current level.

93.215 APPLICATION AND AWARD PROCESS:

Preapplication Coordination:  Preapplication coordination is not requested. This program is eligible for coverage under E.O. 12372, "Intergovernmental Review of Federal Programs." An applicant should consult the office or official designated as the single point of contact in his or her State for more information on the process the State requires to be followed in applying for assistance, if the State has selected the program for review.

Application Procedure:  Grant application Form PHS-5161-1, as approved by the Office of Management and Budget under control number 0937-0188, with all assurances, must be completed and submitted in time for orderly processing.

Award Procedure:  Each application will be evaluated by objective review; conducted in accordance with Public Health Service, Grants Administration Manual, Part 134.

Deadlines:  Contact Headquarters Office listed below for application deadline.

Range of Approval/Disapproval Time:  From 90 to 120 days from date of application receipt.

Appeals:  Applicants may revise and resubmit applications.

Renewals:  Support is recommended for a specific project period, usually not in excess of 5 years. After initial awards are made, projects may be renewed annually depending on the submission and approval of a satisfactory application.

Criteria for Selecting Proposals:  Applications for funding under Section 320(a)(2) must have the capability of managing comprehensive care to: (1) Provide outpatient medical care, management and follow-up to patients and contacts of patients who have Hansen's disease; (2) act as a consultant, community resource and provide in service education to health care providers in the community; and (3) perform or participate in specific clinical evaluations and/or studies related to Hansen's disease.

Examples of Funded Projects:  Hansen's Disease Outpatient Clinics, Hospital Ambulatory Care Centers, Public Health Centers, Community Health Centers, University Hospitals, University Schools of Medicine, Medical Specialty Clinics, State Boards of Health, and State Boards of Health.

Range and Average of Financial Assistance:  The range is from $35,000 to $375,000.

93.215 PROGRAM ACCOMPLISHMENTS:

Eleven centers were funded in fiscal year 1999. Eleven centers are expected to be funded in fiscal years 2000 and 2001.

93.215 ASSISTANCE CONSIDERATIONS:

Formula and Matching Requirements:  Not applicable.

Length and Time Phasing of Assistance:  Awards are made annually. After awards are issued, funds are released in accordance with DHHS payment procedures, which may be through an Electronic Transfer System or a Monthly Case Request System.

Uses and Use Restrictions:  Grants may be used to supplement the cost of providing Hansen's disease diagnostic and treatment services to patients and contacts of patients diagnosed with Hansen's disease.

93.215 POST ASSISTANCE REQUIREMENTS:

Reports:  The following annual program reports are required: Leprosy Surveillance Form, Semi-annual Patient Care Activity Report, Reporting of Patient Deaths, Hand and Foot Screens, Short-Term Protocol Report Forms. An Annual Financial Status Report SF-269, Audits in accordance with OMB Circulars A-128 or A-133, and special reports are required as requested.

Audits:  In accordance with the provisions of OMB Circular No. A-133 (Revised, June 24, 1997), "Audits of States, Local Governments, and Non-Profit Organizations," nonfederal entities that receive financial assistance of $300,000 or more in Federal awards will have a single or a program-specific audit conducted for that year. Nonfederal entities that expend less than $300,000 a year in Federal awards are exempt from Federal audit requirements for that year, except as noted in Circular No. A-133. In addition, grants are subject to inspection and audits by DHHS and other Federal officials.

Records:  Financial records must be kept available for 3 years after the submission of financial status reports, and 3 years after the final disposition of non-expendable property. If questions remain such as those raised as a result of an audit, records must be retained until the problem is resolved.
